Mileage Bonus $.02 per mile
Drivers with over 30,000 miles quarterly will receive an additional $.02 per mile for all miles in that settlement period. Payment is made by the end of the month following the last month of the period. This payment is available only if there are no log violation warnings issued to that driver for the settlement months in which the miles were driven during Styer's initial log audit. Drivers are expected to meet a minimum of 23,000 quarterly.
Recruiting Bonus $.01 per mile
If you recruit a driver, you will be paid $.01 per mile extra, up to one year or until the recruited driver leaves Styer.

DOT BONUS
You will receive a $40 bonus for every Haz Mat DOT inspection with no violations and $25 bonus for every non Haz Mat DOT inspection with no violations.
DETENTION at delivery
$30 per hour after two hours at receiver. This must be documented on the bill of ladings with an arrival time and a departure (empty) time. In addition, dispatch must be notified that you are being detained prior to the two hours window expiring.
LAYOVER
24 Hour Layover $125
16 Hour Layover $100
For LME load $60 (must be approved by dispatch)
Calculated from time of last delivery until the time of your next pickup while outside of MN.
HANDLING $25 per hour to assist in loading or unloading. This must be documented on bill of ladings and preauthorized by dispatch.
CHICAGO CITY PAY – $20 for delivery and pickup inside the tollway perimeter (once per trip)
NEW YORK CITY PAY - $175.00 per trip to NYC Boroughs
CITY PICKUP AND DELIVERY PAY $30.00 per hour when hired to make deliveries and pickups for loads other than your own
PROPANE $10 every time you fill a propane tank. Receipt must be provided.
FUEL SURCHARGE
Styer will pass on 100% of fuel surcharge Styer receives from a customer on each load.
Guaranteed fuel surcharge if Styer’s customer does not pay fuel surcharge
National Avg. $1.46 to 1.50 Contractor gets $.01 per mile
$1.51 to 1.55 Contractor gets $.02 per mile
Increases $.01 per mile with every 5 cent increase in National Avg

OTHER:
· T-Chek transactions will be charged at $1.25 per use plus cost of purchase
· FED EX or UPS on Styer Accounts will be $20 per use or actual cost if higher
· Payments to 3rd parties on behalf of contractors will cost $1 per payment
· Charges on Styer Accounts for contractor purchases will cost $20 per use
· Styer delivery of contractor loads at contractor request or need will be $40 per delivery
· Advances over the $200 provided weekly will incur a charge of $5 per extra amount added for cash advances.
· If your paperwork is missing release numbers from customer, pages of Bill of Ladings, Appointment Unloading Records or Missing Trailer Control Records, $5.00 will be deducted from your settlement to help cover the cost of Styer recovering these documents. If a continued pattern of missing paperwork is detected, settlement will be held until the paperwork is submitted or Styer is paid for the shipment.
· Bonuses may be applied to plate payments. Styer will hold the funds at the original value. Interest will not accrue. Bonuses must be earned prior to the due date for plates in order to be applied to plates.
· Cell phones are expected as means of communicating with Styer. The prior communication contribution is built into the base mileage raise.

Full-time status is given to employees who regularly work over 30 hours per week or 1800 miles per week over a 3 month average

HEALTH/DENTAL INSURANCE
An HMO plan will be offered to full time employees beginning the next 1st of the month after 30 days of employment. Styer pays 100% of employee coverage. The cost for dependents is $85 weekly for spouses and $37.50 weekly for other dependents. Dental premiums cost employees $6.00 per week for all dependents. Health and dental premiums will be paid directly to our health care providers and that portion of your compensation is untaxed. You do have the option to be taxed on premiums.
LIFE INSURANCE
A $10,000 life insurance policy will be provided by Styer Transportation. The plan will be offered after 90 days of employment to workers maintaining full time status.
RETIREMENT PLAN
A 401K plan is available to employees averaging over 30 hours per week. One year of service is required before entry into the plan. Styer contributes 100% of the first 3% of employee contributions and 50% match on the next 2% of employee contributions.
VACATION POLICY:
Pay is based on prior calendar year 52 week average.
After 1 year - 5 days
After 3 years - 10 days
After 7 years - 15 days
After 12 year – 20 days
Full time status is required for vacation benefits. The date used for calculating vacation days is the anniversary date of employment.
You will be paid $50 for each of the following Holidays spent on the road; Independence Day, Thanksgiving and Christmas.
SICK DAYS/PERSONAL DAYS:
Because Styer is a small trucking company, we rely on everyone to be available to work during our regular business days. We do understand, however, that illness to employees and family members occur. There are no specific number of sick days allowed. The days are expected to be taken only when necessary. If it appears that this policy is being abused, a discussion will take place and limits may be placed on these days. If you are away from work for three consecutive days, however, a written notice from a doctor is required.
Requests for personal time will be discussed and evaluated for approval.
DEATH IN THE FAMILY
Time will be allowed to attend funerals and make arrangements if a family member dies. In the case of the death of a spouse or children one week will be provided. Three days will be provided for the death of a parent, sibling or grandparent.
