The following is a brief summary of the benefits that Gulf Coast Transport, Inc. offers. This information is not intended to be a complete description of the plans but will serve as a guide to help you have a basic understanding of our benefits.

Basic Life & AD& D – Basic Life and Accidental Death and Dismemberment provides a benefit to all eligible employees in the amount of a flat $20,000. The Basic Life and Accidental Death and Dismemberment is 100% employer sponsored.

Medical Insurance – We offer two Preferred Provider Organization Medical plans, PPO Core Plan and PPO Buy-up Plan. The major differences between the two plans are the co-pays, deductibles and co-insurance amounts.

Vision – Plan pays maximum of $50.00 annually for eye exams and $150 for glasses or contacts every two years.

Dental Insurance – The Dental benefit provides 100% for Type I Preventive Care Services, 80% for Type II Basic Services, and 50% for Type III Major Services.

401K – Administered by ING. Employees may contribute from 1% to 75% of their gross wages. Company match is 50% of employee contribution up to 4% of gross wages. 100% vested after 6 years of employment.

Voluntary Life – Offered through GE Group Life Assurance Company and may be elected in addition to the Medical/Dental/Life coverage or elected stand alone by all eligible employees. Eligible employees may also elect Dependents Life for spouse and children.

Voluntary Life for Employees – All eligible employees may elect benefits in $10,000 increments up to 5 X salary or a maximum of $250,000. (amounts over $100,000 will require evidence of insurability).

Voluntary Dependent Life for Spouse – All eligible employees may elect Voluntary Dependent Life for their spouse. Benefits may be elected in $5,000 increments to the lesser amount of 50% of the employee benefit or a maximum of $50,000. (amounts over $25,000 will require evidence of insurability).

Voluntary Dependent Life for Dependents – All eligible employees may elect Voluntary Dependent Life for their unmarried dependents. A choice of $5,000 or $10,000 benefit may be elected for dependents between ages 6 months through age 23.
